US President Donald Trump signed an executive order on January 20, 2025, initiating the United States’ withdrawal from the World Health Organization (WHO). This decision revives a similar attempt made during his first term in 2020, which was later reversed by President Joe Biden. 
Reasons for Withdrawal
President Trump cited the WHO’s alleged mishandling of the COVID-19 pandemic and other international health crises as primary reasons for the withdrawal. He accused the organization of failing to act independently from the “inappropriate political influence of WHO member states” and criticized the “unfairly onerous payments” required from the U.S., which he noted were disproportionate compared to contributions from other countries, such as China. 
Implications of the Withdrawal
The United States is the WHO’s largest financial contributor, accounting for approximately 18% of its overall funding. The withdrawal process, which requires a one-year notice period, will halt all U.S. financial contributions to the organization. This move threatens to disrupt critical health programs, particularly those addressing tuberculosis, HIV/AIDS, and other health emergencies. 
Additionally, the executive order mandates the cessation of U.S. participation in negotiations on the WHO pandemic treaty, the recall of U.S. government personnel working with the WHO, and a review of the U.S. Global Health Security Strategy. 
Historical Context
This is the second time in less than five years that the U.S. has initiated a withdrawal from the WHO. In 2020, President Trump announced plans to exit the organization, accusing it of aiding China’s efforts to “mislead the world” about the origins of COVID-19. However, this withdrawal was halted by President Biden upon taking office in January 2021. 
Global Reactions
The WHO has not yet issued an official response to the U.S. withdrawal. However, health experts and international leaders have previously warned that such a move could undermine global health initiatives and weaken the international response to health crises. The loss of U.S. funding is expected to have significant repercussions on the WHO’s ability to combat diseases worldwide. 
Future Considerations
The withdrawal process is set to be completed by January 20, 2026, unless reversed by a future administration. During this period, the U.S. government plans to identify alternative partners to assume roles previously fulfilled by the WHO and to develop a revised Global Health Security Strategy.
